Moxidectin (CAS 113507‑06‑5, C₃₇H₅₃NO₈, MW 639.8) is a macrocyclic‑lactone antiparasitic of the milbemycin family. It is a semi‑synthetic derivative of nemadectin, a fermentation product of Streptomyces bacteria, first commercialised as a veterinary medicine in Argentina in 1990.
Mechanism: it binds to glutamate‑gated chloride channels (and, to a lesser extent, GABA receptors) in invertebrate nerve and muscle cells, causing chloride influx, hyperpolarisation, flaccid paralysis and death of the parasite. Mammals lack these channels in the periphery, giving the class a wide therapeutic index.
Key pharmacology: extremely high lipophilicity → it partitions into fat and skin, forms a tissue reservoir and is released slowly. Terminal half‑life is measured in weeks (roughly 20–23 days in humans after an oral dose; similarly prolonged in animals). This "depot" behaviour is the foundation of almost all its commercial value.
It exists in two separate worlds: a human prescription drug and a large family of veterinary products. They are not interchangeable — veterinary formulations must never be taken by people.

Onchocerciasis (river blindness) caused by Onchocerca volvulus — FDA‑approved 13 June 2018 (NDA 210867, applicant Medicines Development for Global Health), 2 mg tablets given as a single 8 mg dose. Initially for ages ≥12 years; in February 2025 the indication was expanded to children ≥4 years and weighing ≥13 kg. It kills microfilariae but not adult worms, so repeat annual dosing is still needed.
In clinical trials (not approved): scabies (oral 8/16/32 mg dose‑finding), strongyloidiasis (non‑inferior to ivermectin in meta‑analyses), lymphatic filariasis, and soil‑transmitted helminths in combination with albendazole.
Veterinary medicine (the bulk of real‑world use)
Dogs & cats: heartworm (Dirofilaria immitis) prevention; treatment of circulating microfilariae; fleas, ear mites, sarcoptic and demodectic mange, and gastrointestinal nematodes.
