Obeticholic acid can inhibit bile acid synthesis and is used in the treatment of primary biliary cirrhosis and non-alcoholic fatty liver disease.
Obeticholic acid is a farnesoid X receptor (FXR) agonist. FXR is a nuclear hormone receptor that is mainly expressed in the digestive tract's small intestinal epithelial cells, hepatocytes, and bile duct endothelial cells. Obeticholic acid can reduce bile acid synthesis by activating FXR, ultimately regulating bile acid homeostasis and enterohepatic circulation. In conditions of cholestasis, reduces the production of bile from the liver and promotes its excretion, protecting liver cells from excitotoxic damage to bile.
